Sponsorship - Changes Form

Use this form to add students or request changes to an existing Agency Sponsorship Application.

Important Notice:

Please note that beginning in the Spring 2026 term, a late sponsorship fee will be introduced for sponsorship forms submitted after the deadline of May 1, 2026. The fee will be $50.00 for each late submission of up to 10 students. Each additional group of 10 students submitted at the same time would be an additional $50.00

Please note: If your agency chooses not to cover these optional fees, the student remains responsible for payment. Failure to pay these fees will result in a hold being placed on the student’s account until the outstanding balance is cleared.
