Report a Safety Incident - Students

Summary

Instructions for students on how to report an incident that occurred at the University of Saskatchewan.

Body

You must report any occurrence or event that causes or has the potential to cause injury, illness or damage to people, property and the environment that happens while engaged in activities at, or conducting work for the University of Saskatchewan. The reporting process varies depending on whether you are a staff or faculty member, student, or a member of the public.
All incidents, injuries, near misses, property or equipment damage are to be reported to Safety Resources and your supervisor at the time of the incident​​​​​​, or as soon as possible.

Incident Reporting

  1. Go to Report a Safety Incident - Students
  2. Click "Submit Safety Incident Report" in the top right-hand corner to open a blank form.
  3. Complete all fields that have an asterisk "*" as accurately as possible. Include:
    • Date
    • Time
    • Location of incident
    • Names and contact information of those involved and any witnesses
    • Any actions taken immediately following the incident
  4. Submit the form
    The report will be processed by Safety Resources. If anything further is required, you will be notified by email.

Your supervisor/instructor will receive a notification of the incident and will be able to view the information you provided. They are required to add their comments and an action plan to prevent a recurrence. Safety Resources will contact you should any additional documentation be required. 

What is a Safety Incident

What is an incident?

  • An incident is any occurrence or event that may cause injury to a worker or damage to the environment.
  • A "near miss" incident refers to an event that had the potential to cause, but does not actually result in, injury, illness or damage to people, property and/or the environment.

When to report an incident

  • If you're involved in an incident (including near misses) while engaged in activities at, or conducting work at USask, notify your supervisor immediately and report the incident.

Why report an incident?

  • Incident reports document and provide formal records of all incidents that have occurred at USask.
  • The information gathered is important for determining the cause(s) of the incident with the goal of identifying controls that can be implemented to prevent a recurrence.

What happens after you report?

  • Once the report is submitted, Safety Resources will process and review the incident.
  • You may be contacted to provide additional information and/or participate in review meetings to ensure all learnings are captured, and get your feedback on preventing a recurrence.

Who should report?

  • Incident reports must be completed by the individual involved in the incident. This should be supported by the individual's direct supervisor.
