Incident Reporting

What is an incident?

  • An incident is any occurrence or event that may cause injury to a worker or damage to the environment.
  • A "near miss" incident refers to an event that had the potential to cause, but does not actually result in, injury, illness or damage to people, property and/or the environment.

When to report an incident

  • If you're involved in an incident (including near misses) while engaged in activities at, or conducting work at USask, notify your supervisor immediately and report the incident.

Why report an incident?

  • Incident reports document and provide formal records of all incidents that have occurred at USask.
  • The information gathered is important for determining the cause(s) of the incident with the goal of identifying controls that can be implemented to prevent a recurrence.

What happens after you report?

  • Once the report is submitted, Safety Resources will process and review the incident.
  • You may be contacted to provide additional information and/or participate in review meetings to ensure all learnings are captured, and get your feedback on preventing a recurrence.

Who should report?

  • Incident reports must be completed by the individual involved in the incident. This should be supported by the individual's direct supervisor.
     

How to Report

1. Fill out the incident report form, as completely and accurately as possible.

2. Forward to safetyresources@usask.ca.

It is the responsibility of supervisors to provide safe working conditions and ensure that actions are taken to prevent similar incidents or injuries from recurring.

Incident reporting is available online 24/7 along with emailing the safety resources general email with general questions. Alternatively, in an emergency or after hours call 306-966-5555 and Protective Services would be able to support/direct from there
