How to Complete the “Agency Sponsorship Application Form”

Who should complete this form?

This form is for is for sponsorship agencies who agree to pay for individual student(s) tuition and/or fees. The form needs to be completed by an individual in the organization with signing authority. Students are not eligible to submit this form on a sponsor’s behalf. This form needs to be submitted directly by the sponsoring agency.

Instructions

Step 1: Access the Form

 

Step 2: Complete the Sponsor Information Section

  • Agency Name – Enter the full legal name of the sponsoring organization that you have signing authority for. 
     
  • Vendor ID – Enter your Vendor ID. This can be found on a previous invoice or correspondence.  
     
    • If this is your organization’s first time sponsoring a student at Usask, do not complete this form. Instead, email agencysponsorship@usask.ca to be set up with a Vendor ID first. You cannot complete this form without a Vendor ID.
       
  • Sponsor Type – Select the appropriate sponsor type (e.g., Indigenous community, Saskatchewan Government, or Other).
     
  • Contact Name, Role, Phone and Email – enter all contact information including name, role, phone number, and email address. This is the email where invoices will be sent.
     
    • If your Sponsor Type is “Indigenous Community”, two “contact” boxes will appear for “Education Contact” and “Finance Contact”. Please fill out each contact’s name, role, email address and phone number.
       
  • Billing Address – Enter the full mailing address (street, city, province, postal code, country).
     
  • Fax – Optional.

 

Step 3: Complete the Authorized Coverage Section

  • Term – Select the academic term (e.g., Winter 2026, Spring 2026).
     
  • Are you paying full tuition? – Select Yes if there is no maximum dollar amount that your organization is covering for your sponsored student(s). 

If you select Yes, four optional student fees will appear. Choose which fees your agency will cover:

Students may opt out of the Health and Dental Plan if they can provide proof of equivalent coverage through a parent’s or spouse’s insurance plan, or if they are covered by the Non-Insured Health Benefits (NIHB) program. They may also opt out of the U-Pass if they live outside the city or meet other approved exemption criteria.

  • If your agency is not paying a student’s full tuition and there is a maximum sponsorship dollar amount, select No.

    You will need to indicate the maximum amount of sponsorship on the attached Excel spreadsheet.

 

Step 4: Review the Additional Information and Authorization Section

  • Check the Authorization box to confirm that you understand and agree to the terms and conditions.

 

Step 5: Attachments

  • Upload the Excel spreadsheet containing all sponsored students. Ensure that each student’s name, student ID number, and maximum coverage amount (if applicable) are included.

 

Step 6: Submit the Form

  • Review all fields to ensure the information is accurate.
     
  • Make sure to click Submit at the bottom of the form to ensure your application is submitted successfully.

Important Notes

  • A new form must be submitted for each academic term.
  • Submit a Sponsorship Changes form if you need to add students or make updates to a previous application.
  • Submit a Sponsorship Late Application if the deadline for sponsorship submissions has passed.
  • Incomplete or inaccurate information may delay processing.
  • Students’ privacy is protected at USask, and information is unable to be shared with sponsoring agencies regarding student grades, tuition and fee information aside from sponsored tuition, etc, without authorization. If you are interested in this information, students have the option of setting up Proxy. Proxy allows students to share certain information (view-only access) with a person or organization.
